Kings County Department of Public Health - Vital Statistics Unit registers all births and deaths that occur in Kings County.

Vital statistics can also issue certified copies of birth and death from previous to present year. All other years can be obtained from the county clerk recorders office at: http://www.countyofkings.com/acr/clerk/index.html.

Vital statistics registers all out of hospital births that occur before the child turns 1 year of age. We also provide pamphlets and forms for any amendments or changes needed to be made to a birth or death certificate.

Vital Statistics can witness signatures to complete the Declaration of Paternity Form.

  • To obtain a certified copy of a birth or death certificate, you must complete an application with all the requested information completed.
  • If requesting in person, an authorized person must present a picture ID and sign a statement sworn under penalty of perjury.
  • If requesting by mail, a notarized statement sworn under penalty of perjury located at bottom of application must be completed
