REQUIREMENTS FOR OBTAINING A PASSPORT

For STANDARD SERVICE
(Average Processing Time 6 Weeks):

Please Bring the following items with you:

1)  Certified Birth Certificate OR an Original Naturalization       Certificate (These will be returned to you with your       passport)

2)  Two Passport Photos (2x2 in size)

3)  Driver's License or Government Issued Photo ID Card

4)  A Check OR Money Order in the Amount of:
              $110.00 FOR ADULTS (16 AND OLDER) OR
              $80.00 FOR A MINOR UNDER THE AGE OF 16
        PAYABLE TO:     DEPARTMENT OF STATE

5)  $25.00 Processing Fee (Per Application)
        PAYABLE TO:  KINGS COUNTY CLERK RECORDER
         (Check, Money Order or Cash Accepted)

* As of February 1, 2008 both parents must appear to sign the application for children under the age of 16. Please ask us for additional information. (ALL CHILDREN MUST BE PRESENT)
