Please fill out the form on screen, upload photo of appropriate license card into the form, then print the form and mail the printed forms (wet signatures are required) with your check back to: Kings County Dept. of Agriculture, 680 N. Campus Drive, Suite B, Hanford, CA 93230. (Registrations will not be valid unless the fees are also paid). Your proof of registration and receipt will be returned to you in the mail.

Farm Labor Contractor (FLC): 2019 Form

Document required: Farm Labor Contractor License (from Department of Industrial Relations)-should have picture, current address, etc. Read, sign, date the “Kings County Farm Labor Contractor Registration Information.”
Payment for registration $50.00 - receipt will be given/mailed. Registration by mail or in-person; may send an authorized person (with authorization letter) to register on FLC’s behalf. Print and sign, mail original wet signature copy with your check. The A-9 is required to be posted at all work sites: Fieldworker Posting A-9 - (Download the A-9 Document in English) or (Download the A-9 Document in Spanish)

Letter of Authorization: Form

Maintenance Gardener Business (MG): Form
Documents required: Qualified Applicator License (QAL) or Qualified Applicator Certificate (QAC), and Business License, issued by Dept. of Pesticide Regulation (required for both QAL/QAC).
Payment for registration:  $25.00 for QAC for Maintenance Gardener- receipt given /mailed.
Print and sign, mail original wet signature copy with your check.

Pest Control Business (PCB): Form
Documents required: Qualified Applicator License (QAL) or Qualified Applicator Certificate (QAC), and Business License, issued by Dept. of Pesticide Regulation (required for both QAL/QAC). Completed Equipment List form. Medical Supervision form if necessary.
Payment for registration: $50.00 for QAL (full Ag. PCB), or $25.00 for QAC for Maintenance Gardener- receipt given /mailed. Registration for home county must be in-person. Out of county may be by mail or in-person.
If PCB is using restricted pesticides or managing farm properties they may need a Restricted Materials Permit.
Print and sign, mail original wet signature copy with your check.

Pest Control Advisor (PCA): Form    
Upload a photo of your valid Pest Control Advisors License (PCA) on to the form.
Print and sign, mail original wet signature copy with your check. Payment of $10.00 if home county (where your records are kept), or payment of $5.00 if from out-of-county. In-person is required for home county registration.(FAC 12301)
Out-of-county registration by mail or in-person, must provide proof of home county registration.

Pest Control Advisor (PCA) Signature Authorization: Form

est Control Aircraft Pilot (Pilot): Form  
Upload a photo of your valid DPR issued Pest Control Aircraft Pilot License (Journeyman “JP” or Apprentice “AP”)
Print and sign, mail original wet signature copy with your check. If the pilot is an Apprentice, he/she must work under the supervision of a Journeyman Pilot. The Journeyman Pilot must already be registered with the county in order for Apprentice to register. You must designate who your Journeyman Pilot Supervisor is, on your registration form.
Payment of $10.00 if home county, or payment of $5.00 if additional county (must provide proof).
In-person required for home county registration. Out-of-county registration by mail or in-person (must provide proof of home county registration).

Structural Branch 1: Form    
Must list OPR and FR licensees - list of applicators is required with registration. Payment of $25.00 – receipt will be given/mailed. Payment of $35.00 for all Branches. Registration by mail or in-person.

Structural Branch 2/3 (2 or 3, or 2 & 3 together): Form
Must list licensed Qualifying Manager. Payment of $10.00 -  receipt will be given or mailed. Registration may be accomplished by mail or in-person.
