Division of Nursing and Community Services

Public Health Nurses in Kings County are generalists.  Services are provided for a range of ages from birth to the elderly for all kinds of medical issues.  Assessments are done through home visiting.  If a case is opened, it will be managed according to Targeted Case Management guidelines, including referrals to appropriate services.  Coordination of services is provided for the client with the medical provider, social services, school personnel, early intervention services, and other appropriate agencies.

The Nurse of the Day (NOD) provides nursing consultation and services to the community, medical providers, and other agencies by phone or walk-in basis.  Service is available from 8:00 A.M. to 5:00 P.M.

Referrals can be made to the Nurse of the Day by calling 1-800-649-5399 or by completing the referral form (here) and faxing to (559) 584-5672 addressed to the Public Health Nursing Supervisor.
